この書類とは / What is this form

「事業開始等申告書」は、東京都内で個人事業を始めたことを都税事務所に届け出る書類です。個人事業税の課税対象を都道府県が把握するために使われます。

The Business Commencement Notification reports to the Tokyo metropolitan government that you have started a business — used to assess individual enterprise tax (個人事業税).

いつ必要か / When you need it

東京都内で個人事業を開始したときに提出します（他の道府県にも同様の届出があります）。

File it when you start a business in Tokyo. Other prefectures have an equivalent notification.
